POSTERIOR TIBIAL TENDON DYSFUNCTION (PTTD) or ADULT ACQUIRED FLATFOOT

ANATOMY

The posterior tibial tendon is one of the major supporting structures of the foot.  The tibial tendon is located in the posterior (back) of your leg.  The tibia is one of the two bones of your lower leg.

MECHANISM OF INJURY

This condition can occur through a variety of physical activities that are either performed improperly or with excess.  Some examples can include running, swimming, dancing or simply walking on the innermost portion of your foot.  The muscles in the foot compensate which causes the strain.  It can also be the result of trauma to the lower part of your body like a car accident or other abrupt motion.  Arthritis can also induce posterior tibial tendonitis. 

SYMPTOMS

-Pain and edema near the arch of the foot.

-Pain and edema to the inner side of the ankle.

-Increase in severity of pain when you try to rise up on the ball of your foot.

-Increase in severity of pain when you extend your foot in an upward position.

-As PTTD advances, the arch flattens more and the pain shifts to the lateral (outside) of the foot below the ankle.
